Like most health care practices, we have policies, procedures and patient responsibilities. Their purpose is to enable us to best serve you, and our community as a whole!
1. Making Appointments: For healing to be most effective, the Doctor usually suggests a series of visits. We advise that you schedule in advance to ensure continuity of appointments.
2. Appointment Cancelation: Your appointments and well-being are very important to us. We understand that sometimes, the unexpected can occur. If you need to cancel your appointment, we respectfully request at least 48-hours notice. Advanced notice will allow another patient access to that appointment time.
3. Late Cancelation & No-Show: A cancellation is considered “late” when the appointment is canceled less than 48-hours before the scheduled time. A “no-show” is when a patient misses an appointment without canceling. In either case, we will charge the patient the full cost of the visit. If you are more than 10 minutes late for your service, we may not be able to accommodate you. In this case, the same cancellation fee will apply. We will gladly reschedule your service for another time that is convenient to you. If you miss more than 3 appointments in a row, we reserve the right to cancel future appointments until care can be consistently re-established.
4. Payment Policy: Co-payments, deductibles and non-covered services and goods, including but not limited to supplements and prescribed equipment, are due at the time of service. Please note, special orders require payment in advance.
5. Collections Policy: Past due balances need to be paid in full prior to scheduling your next appointment unless other arrangements are made. Balances carried beyond 60 days will be assessed 1.75% interest, per month, until paid in full. Any payment returned for insufficient funds, closed accounts or other issues may be subject to a $35 service fee.
6. Change of Information: Please notify us if your name, address, phone number or insurance information has changed, as soon as possible.
7. Electronic Devices: That we may provide you the best possible care, we ask you to turn off your personal communication devices before entering the treatment room.
